How it works

Lactitol
In the colon it is broken down to short chain organic acids and increases osmotic pressure in the colon, thereby causing an increase in the stool water content and stool volume. a majority of ingested lactitol reaches the large intestine, where it then becomes fermentable to gut microbes (prebiotic) and can pull water into the gut by osmosis. The microbial fermentation leads to acidification and subsequent reduction of ammonia absorption. Lactitol in combination with Ispaghula husk is an approved combination for idiopathic constipation as a laxative and is used to prevent or treat constipation.
Ispaghula
It is capable of absorbing 40 times of its own weight in water and so can markedly decrease stool viscosity. Ispaghula husk consists of 85 % water soluble fibre; it is partly fermentable (in vitro 72 % unfermentable residue) and acts by hydration in the bowel. Gut motility and transit rate can be modified by ispaghula husk through mechanical stimulation of the gut wall as a result of the increase in intestinal bulk by water and the decrease in viscosity of the luminal contents. When taken with a sufficient amount of liquid (at least 30 ml per 1 g of herbal substance) ispaghula husk produces an increased volume of intestinal contents due to its highly bulking properties and hence a stretch stimulus, which triggers defaecation; at the same time the swollen mass of mucilage forms a lubricating layer, which makes the transit of intestinal contents easier. The purely mechanical action of psyllium mucilage absorbs excess water while stimulating normal bowel elimination. Although its main use has been as a laxative, it is more appropriately termed a true dietary fiber and as such can help reduce the symptoms of both constipation and mild diarrhea.
The combined action of Lactitol & Ispaghula results in faster and shorter transit time in passing stools.

Dosage

15 gm once daily with plenty of water
40gm Lactitol can be given in a day.

Side Effects

Ispaghula
Hypersensitivity, such as allergy
Excess gas in the stomach and intestines (flatulence or wind).
Bloating or swelling of the abdomen.
Obstruction of the intestines if the medicine is not taken with sufficient fluid (see the instructions on how to take above).
Lactitol
At the start of treatment, Lactitol may produce abdominal discomfort mainly flatulence and seldom abdominal pain or sometimes an abdominal distension. Such effects tend to diminish or disappear after a few days of regular intake of Lactitol.
Gastrointestinal disorders: Very rare: vomiting, abdominal pain, abdominal discomfort, abdominal distension, diarrhea, nausea, flatulence. Not known: gastrointestinal sounds abnormal.

Contraindication

Ispaghula
Ispaghula husk should not be used by patients with a sudden change in bowel habit that persists for more than 2 weeks, undiagnosed rectal bleeding and failure to defaecate following the use of a laxative. Ispaghula husk should also not be used by patients suffering from abnormal constrictions in the gastro-intestinal tract, with diseases of the oesophagus and cardiac, potential or existing intestinal blockage (ileus), paralysis of the intestine or megacolon, diabetes mellitus, which is difficult to regulate. This product should not be taken by patients who have difficulty in swallowing or any throat problems. Patients with known hypersensitivity to the active substance should not use ispaghula husk and its preparations.
Lactitol
Hypersensitivity to lactitol or to any of the excipients. Lactitolproduces its effect in the colon and is therefore contra-indicated in all cases where intestinal transit is not assured (intestinal occlusion, etc.). As with all laxatives, Lactitol should not be used in the case of any symptoms or suspicion of organic lesion of the digestive tract and in the case of any unexplained abdominal pain or rectal bleeding. Faecal impaction should be treated using alternative means prior to the use of laxatives. Breastfed babies and infants with autosomal recessive hereditary fructose intolerance. Lactitolis contra-indicated in galactosaemia. The incomplete metabolism of lactitol may result in the development of fructosemia and galactosaemia and their sequelae. Preexisting fluid electrolytes imbalance.
USFDA pregnancy category B. Lactitol may not cause harm to an unborn foetus. Before Lactitol treatment, the patient should discuss with the physician, if they are planning for a pregnancy.
Nursing mothers should consult a physician before taking Lactitol
